PeKa B40, on the other hand, is specifically for households in the bottom 40% income bracket and provides similar hospitalisation coverage but with more flexible access. Both are complementary to private insurance rather than replacements for it.

MySalam covers hospitalisation at government facilities, but private insurance fills the gaps—like outpatient care, specialist consultations, and treatment at private hospitals where waiting times are shorter. Many Malaysians use MySalam as their base layer and add private insurance for better flexibility and faster access to care.

A private hospital stay in Malaysia typically costs RM3,000–RM8,000 per day depending on the ward type and hospital, plus additional costs for surgery or ICU care. For a 5-day stay with moderate procedures, you might need RM20,000–RM50,000 in reserves. That’s why insurance and government schemes matter—they cover a significant portion so you’re not caught off-guard.

MySalam and PeKa B40 don’t cover outpatient visits, prescription medications, dental work, or optical care. A specialist consultation can cost RM150–RM300, while chronic disease medications add up quickly. This is where personal budgeting and supplementary insurance plans become essential for managing day-to-day healthcare expenses.

Look at three key areas: annual coverage limits, co-insurance percentages (what you pay vs. insurer), and exclusions. Check whether the plan covers pre-existing conditions and what the waiting period is. Get quotes from at least 3 providers, compare renewal costs over 5 years, and ensure the plan covers your preferred hospitals.

PeKa B40 eligibility is based on your household’s monthly income. If you’re at the threshold, it’s worth checking with your local Jabatan Kebajikan Masyarakat (Social Welfare Department) for a formal assessment—income thresholds vary slightly by state. Even if you’re just above the limit, having basic private insurance is still more affordable than facing unexpected medical bills.
